Fast and Easy Enrolment
Setting up a Private Health Services Plan (PHSP) is easy. All Canadian businesses are eligible to set up the plan. Simply print out the appropriate enrolment form and complete. The employer needs to sign. The employer submits the enrolment fee payable to HealthSAVE Inc. for the one time set up fee. There are no annual premiums.
In addition, please complete Appendix A. This is required for both corporations and sole proprietorships. All employees must be declared and the plan must be offered to them.
Send the signed enrolment form, Appendix A for Employees, and cheque for enrolment fee amount to HealthSAVE Inc. processing offices. We process enrolment forms promptly, at least once weekly.
If special arrangements are required, please contact your servicing representative or email our offices. For instance, as claims cannot be backdated, you may wish to fax in your enrolment for same day processing. Please contact us to arrange.
